The dissertation presents a new approach to solving an urgent scientific problem – improving the results of treatment of patients with gallstone disease (GD) combined with gastroesophageal reflux disease (GERD) and hernia of the esophageal orifice of the diaphragm (HEOD) using an advanced algorithm gallbladder and esophagogastric junction and original minimally invasive methods of surgical correction, which reduces the frequency of postoperative complications, recurrences, and improves treatment outcomes. The aim of the study is to improve the results of diagnosis and treatment of patients with housing and communal services, combined with GERD and HEOD by optimizing the diagnosis, choice of surgical tactics and determination of indications for simultaneous laparoscopic interventions.

The thesis presents a theoretical substantiation and solution of an urgent problem of modern medicine - determination of the features of the combined course of gastroesophageal reflux disease and autoimmune thyroiditis in young people, based on a comprehensive assessment of disorders from the immune status, indicators of the antioxidant system, autoimmune changes and pathomorphological manifestations.

Karazin Kharkiv National University of the Ministry of Education and Science of Ukraine, Kharkiv, 2021. Regional features of sensitizing profile based on the multiplex determination of sIgE (ALEX-test) in patients with allergic diseases living in the western regions of Ukraine were identified and a comparative analysis was performed depending on the patients’ place of residence (Lviv and other oblasts of western Ukraine, urban / rural). Regional and molecular endotypes of different nosologies of allergic diseases were distinguished as well as threshold values of sensitization with regard to causative allergens. Molecular-prognostic models of personalized risk of persistent allergic diseases development (86.5%, p <0.001), intermittent allergic diseases (75.0%, p <0.01), BA (84.6%, p <0.001) have been identified for the patients with the active phase of EBV-infection.

The thesis is dedicated to the improvement of the efficiency of treatment and forecasting of the risks of transformation of reactive arthritis (ReA) into rheumatoid arthritis (RA) through the study of the peculiarities of immunopathogenesis in the background of Chlamydia trachomatis (C. trachomatis) and Epstein-Barr virus (EBV). The rate of activity of the inflammatory process under the DAREA index was higher in patients with RеА with combined infection as compared to the group of patients with RеА with C. trachomatis (p<0,05). Molecular and genetic studies have been established: the detection of the DNA of C. trachomatis (in blood serum and in the urinogenital scraping) and Epstein-Barr virus (in blood, saliva, mucous membrane of the posterior pharyngeal wall scraping) as well as specific molecules of immunoglobulin М and immunoglobulin G (IgМ, IgG) to causative antigens.

Прогнозування перебігу серцевої недостатності у хворих на ішемічну хворобу серця у поєднанні з цукровим діабетом 2-го типу

An examination of 100 male patients with chronic heart failure (CHF) of ischemic genesis and coronary heart disease (CHD) in combination with type 2 diabetes mellitus (DM) was performed. All patients were examined on the basis of the internal medicine department of the municipal non-profit enterprise City Clinical Emergency Hospital named after Prof. A. I. Meshchaninov of the Kharkiv City Council, after a course of treatment in a hospital. Inclusion criteria: age from 50 to 70 years old, CHF class I-II according to NYHA classification (New York Heart Association), LV EF ≥ 50 % (according to the criteria of the European Society of Cardiology, 2016), post-infarction cardiosclerosis, in combination with type 2 DM (according to the criteria of the American Diabetes Association (ADA), with a glomerular filtration rate ≥ 50 ml / min / 1.73 m2, NT-proBNP ≥ 125 pg/ml.
Оптимізація алгоритму серологічної діагностики алергійних захворювань у пацієнтів з полісенсибілізацією

Оптимізація алгоритму серологічної діагностики алергійних захворювань у пацієнтів з полі сенсибілізацією. – На правах рукопису. Дисертація на здобуття наукового ступеня кандидата медичних наук за спеціальністю 14.03.08 – імунологія та алергологія. – Харківський національний університет імені В.Н. Каразіна Міністерства освіти і науки України, Харків, 2020. Дисертацію присвячено питанням оптимізації алгоритму серологічної діагностики алергічних захворювань у пацієнтів з полісенсибілізацією для призначення адекватної специфічної імунотерапії. На базі обстеження 4596 пацієнтів з респіраторною та харчовою алергією виявлено спектр алергенів, які мають пріоритетне значення у Південному регіоні України. Визначено епідеміологічні аспекти CCD-позитивності та вплив антитіл до бокових карбогідратних ланцюгів алергенів (CCD) на популяційний профіль сенсибілізації пацієнтів з алергічними захворюваннями та виявлено кореляційний зв'язок між кількістю позитивних реакцій на досліджувані алергени та відсотком пацієнтів, які реагували позитивно на ССD-маркер. Встановлено, що пацієнти з наявністю анти-ССD антитіл значно змінюють епідеміологічну картину сенсибілізації в популяції за рахунок підвищення проценту позитивності до рослинних та інсектних алергенів, а також латексу. Обґрунтована доцільність використання CCD-блокерів у пацієнтів з полісенсибілізацією для оптимізації алгоритму серологічного обстеження таких пацієнтів. На базі використання CCD-блокерів запропонований алгоритм обстеження та подальшого призначення молекулярної діагностики.
Стимуляція регенерації імунної системи опромінених мишей трансплантацією гемопоетичних стовбурових клітин і прогеніторів, преінкубованих з мультипотентними стромальними клітинами тимуса

Дисертація присвячена вивченню значення контактної взаємодії МСК і ГСК у стимуляції імунологічної, регенеративної і радіозахисної активності трансплантованих клітин. Досліджена відповідна активність накопичених в клітинній культурі МСК тимуса. Експерименти проведені на 519 мишах лінії СВА. Вперше встановлено, що МСК тимуса та гемопоетичні клітини мають мембранну спорідненість, яка забезпечує можливість міжклітинної контактної взаємодії in vitro, що проявляється формуванням фібробласто-лімфоцитарних розеток (ФЛР). Вперше показано, що трансплантовані летально опроміненим мишам преінкубовані з МСК тимуса ККМ і КФП, а також МСК тимуса більш суттєво, ніж нормальні клітини, стимулювали регенеративну активність імунної системи, що проявлялося підвищенням виживаності опромінених тварин і значним відновленням природного і адаптивного імунітету.
